While it is our goal to provide accurate information, the composition of our provider network is subject to change without notice. The participation of a particular provider is never guaranteed, even if a member is referred by their physician to the provider. CHC is in the process of negotiating with additional physicians and hospitals, and changes will be made as appropriate. It is important to know when you enroll in this plan, services are provided through the plan's delivery system, but continued participation of any one doctor, hospital, or other provider cannot be guaranteed. Out-of-Area Providers: PPO Members For PPO members living outside the CHC of Iowa service area, CHC of Iowa is transitioning from the PHCS network to the Coventry Health Care National Network. Starting October 1, 2005, upon your group’s renewal with CHC of Iowa, all PPO members living outside the CHC of Iowa service area will use the Coventry Health Care National Network.
